dc.description.abstractΕισαγωγή Tα τελευταία χρόνια έχουν αναπτυχθεί αρκετές κλίμακες έγκαιρης αναγνώρισης και βαθμολόγησης κινδύνου με σκοπό να βοηθήσουν ιατρούς και νοσηλευτές κατά την υποδοχή των ασθενών στο τμήμα επειγόντων περιστατικών. H χρήση δεικτών έγκαιρης προειδοποίησης (early warning systems – EWS) στα νοσηλευτικά τμήματα, ΤΕΠ συμβάλλουν στην έγκαιρη αναγνώριση (early recognition) και έγκαιρη παρέμβαση (early intervention) στους ασθενείς που αναπτύσσουν οξεία νόσο. Σκοπός είναι η εφαρμογή και συγκριτική αξιολόγηση της κλίμακας MEWS με την κλίμακα NEWS στους ασθενείς που προσέρχονται στο ΤΕΠ σε σχέση με την έκβαση της πορείας τους όπως αυτή εκφράζεται από τη θνητότητα στο νοσοκομείο, τη διάρκεια νοσηλείας και τυχόν εισαγωγή στη ΜΕΘ. Υλικό και μέθοδος Πρόκειται για μια προοπτική μελέτη 2 μηνών, η οποία πραγματοποιήθηκε στο Πανεπιστημιακό Γενικό Νοσοκομείο Ηρακλείου και στο Βενιζέλειο Γενικό Νοσοκομείο Ηρακλείου, όπου έγινε δειγματοληψία κατά τις εφημερίες των ΤΕΠ με τυχαίο ημερολογιακό πλάνο στο οποίο ελήφθησαν υπόψιν όλες οι εισαγωγές των ΤΕΠ από το παθολογικό, χειρουργικό και καρδιολογικό κατά την ημέρα παρατήρησης. Λήφθηκαν οι τιμές των φυσιολογικών παραμέτρων και εξήχθη η βαθμολογία στις δύο κλίμακες. Έπειτα παρακολουθήθηκε η πορεία και η έκβαση των ασθενών. Συμπληρώθηκαν οι κλίμακες MEWS και NEWS καθώς και ένα δελτίο δημογραφικών και κλινικών δεδομένων. Η παρατήρηση συνεχίστηκε καθ όλη την διάρκεια της νοσηλείας έως την τελική έκβαση (εξιτήριο ή εκδημία), ν=200 ασθενείς (100 από κάθε νοσοκομείο). Αποτελέσματα Η μέση ηλικία των ασθενών που προσήλθαν στα ΤΕΠ και έκαναν εισαγωγή είναι τα 62,2±20,2. Οι ώρες εισαγωγής των ασθενών στα ΤΕΠ ήταν κυρίως τις πρωινές ώρες μεταξύ 7:01-15:00 (59,5%). Στον Παθολογικό τομέα έκαναν εισαγωγή το 82,5% των ασθενών ενώ στον Χειρουργικό τομέα (17,5%). Το 15,5% ήταν ασθενείς με κάποια λοίμωξη αναπνευστικού συστήματος, 11% με οξύ στεφανιαίο σύνδρομο, 9% με δύσπνοια, 8% με κάποιο λιποθυμικό επεισόδιο κ.α. Η Μέση Διάρκειας Νοσηλείας (ΜΔΝ) των ασθενών ήταν 6,8 μέρες. Από αυτούς 8 ασθενείς έκαναν εισαγωγή στη ΜΕΘ όπου η μέση διάρκεια νοσηλείας ήταν 4,4 μέρες. Η θνητότητα στη μελέτη ήταν 5%. Καθώς οι ασθενείς αξιολογήθηκαν με τις δύο κλίμακες Early Warning Score (MEWS & NEWS) η μέση τιμή και για τις δύο κλίμακες ήταν: MEWS=2,17 με μέγιστη 6 και NEWS=2,34 με μέγιστη 11. Βρέθηκε ότι η κλίμακα MEWS συσχετίζεται ως προς την διάρκεια νοσηλείας σε αντίθεση με την κλίμακα NEWS όπου δεν παρατηρείται καμία σημαντική συσχέτιση. Επιπλέον ως προς την εισαγωγή στη ΜΕΘ και την θνητότητα και οι δύο κλίμακες σχετίζονται ισχυρά. Για κάθε πρόσθετη ημέρα νοσηλείας ο λόγος πιθανότητας σε σχέση με την θνητότητα αυξάνεται 1,07 (p=0,029). Όσον αφορά την ηλικία για κάθε έτος μεταβολής παρατηρείται σημαντική αύξηση του λόγου πιθανοτήτων (Odds Ratio) σε σχέση με την θνητότητα κατά 1,12 (p=0,007). Για κάθε πρόσθετη ημέρα νοσηλείας ο λόγος πιθανοτήτων σε σχέση με τη θνητότητα αυξάνεται 1,48 (p=0,015). Συμπεράσματα Τα EWS (Early Warning Score) μπορούν να χρησιμοποιηθούν ως εργαλείο για τον εντοπισμό ασθενών που απαιτούν εισαγωγή στο νοσοκομείο και έχουν αυξημένο κίνδυνο θανάτου καθώς επίσης μπορούν να χρησιμεύσουν και ως εργαλείο ταξινόμησης όταν αυτό κρίνεται απαραίτητο. Και οι δύο κλίμακες έχουν θέση στα ΤΕΠ. Εναπόκειται στους υπεύθυνους κάθε νοσοκομείου ποια θεωρούν ότι είναι πιο λειτουργική κατά τόπους.el
dc.description.abstractIntroduction In the last years a few scales for the early recognition and rating of risk have been developed, aiming at helping doctors and nurses during the admittance of patients into the emergency departments. The use of early warning systems (EWS) in the nursing departments, emergency departments, contribute to the early recognition and early intervention to patients who develop an acute disease. Aim The aim is the implementation and benchmarking of the MEWS scale with the NEWS scale in patients that come to the emergency department in relation with the outcome of their course as it is expressed by the mortality rate in the hospital, the duration of the hospitalization and an admission to ICU (intensive care unit). Material and method It is a 2-month prospective study, which was carried out at the University Hospital of Heraklion and the Venizelion General Hospital of Heraklion, where there was random sampling during the shifts of the emergency departments during which all admissions to the emergency departments in the pathological, surgical and cardiological departments were taken into consideration at the day of observance. The normal rates of the parameters were taken and the marking came to two scales. Then the course and the outcome of the patients were observed. The scales MEWS and NEWS were completed as well as a demographic and clinical data report. The observation continued throughout the hospitalization until the final outcome (discharge or demise), ν=200 patients (100 from each hospital). Results The average age of the patients that came to the emergency departments and were admitted is 62,2±20,2. The times of admission of the patients into the emergency departments were usually morning hours between 07:01-15:00 (59,5%). In the pathological sector 82,5% of the patients were admitted while in the Surgical sector 17,5%. 15,5% of the patients suffered from infections of the respiratory tract, 11% from a severe coronary syndrom, 9% with dyspnea, 8% with fainting etc. Τhe average duration of hospitalization of the patients was 6,8 days. Of these, 8 patients were admitted into the Critical Care Unit where the average duration of hospitalization was 4,4 days. The mortality rate of the study came to 5%. As the patients were assessed with the two scales Early Warning Score (MEWS & NEWS) the average rate for both scales was : MEWS=2,17 with maximum 6 and NEWS=2,34 with maximum 11. It was found that the MEWS scale relates to the duration of hospitalization in contrast with the NEWS scale where no significant relation has been noticed. Moreover, as for the admission into the Critical Care Unit and the mortality the two scales are strongly related. For every extra day of hospitalization the odds ratio in relation to the mortality is increased 1,07 (p=0,029). As for the age for every year of change a significant increase of the odds ratio has been noticed, in relation to mortality by 1,12 (p=0,007). For every extra day of hospitalization the odds ratio in relation to mortality increases 1,48 (p=0,015). Conclusions EWS (Early Warning Score) can be used as a tool for the identification of the patients that require admission into hospitals and face a higher risk of demise and they can also be used as a tool for classification when necessary. Both scales hold a position in the emergency departments. It is up to the people in charge of each hospital which one they consider more functional.en
